WHO QUALIFIES FOR HOME HEALTH?
 |
Any
Person Who:
· Has
an illness that requires the skilled services of a nurse
or therapist
· Is under a plan of care that is ordered by a physician
· Meets guidelines mandated by Medicare, Medicaid, or any other private
insurance provider
Referrals can be made by anyone; however,
a physician’s order
is required.

WHAT WILL INSURANCE COVER?
 |
Medicare
and Medicaid usually cover the costs of our services as well
as most medical
equipment and medical supplies. Your nurse will tell you
what supplies are not covered.
Some private insurances
cover home health services. We bill private insurance companies
and then inform
you what is covered and what is not covered. |
SKILLED NURSING
Nurses
can:
· Provide care
that is ordered by your doctor such as catheter and
wound care,
injections, etc.
· Help the homebound person meet their health care needs
· Monitor problems and report the findings to the doctor
· Teach and counsel patients and their family about health care matters
· Collect specimens / labs for physicians |

We have pediatric and maternal nurses,
nurses trained in the use of the wound vac,
nurses trained in ostomy/stoma
care, and
nurses trained to assist with IV therapy in the home.
HOME HEALTH AIDES
|
Aides
can:
· Help a patient bathe
· Help a patient with skin care
· Work under the direction of a therapist or nurse to help the patient
become independent with activities of daily living such as walking, getting dressed,
eating meals, and more
· Report any changes in health status to the nurses

OCCUPATIONAL THERAPY
Therapists
can:
· Work with
a patient after a stroke or accident to help regain strength
in the hands and
arms
· Teach the patient how to use health care equipment
· Show how to keep a home as safe as possible for the patient
· Offer ADL & HDL
training
· Help with Muscle Re-education
· Provide Cognitive Training
· Help With Perceptual Motor Training & Fine Motor Coordination
· Deliver Therapeutic Exercise
· Apply Splints & Joint Mobilization

SPEECH THERAPY
 |
Therapists
can:
· Evaluate speech, language,
and hearing
· Start treatment programs to improve the patient’s speech and language
· Teach the patient and their family various ways to improve speech and
language
· Work with patients who have problems with swallowing

PHYSICAL THERAPY
Therapists
can:
· Consult with a physician to
set goals and to determine the best treatments for improving
the patient’s ability to mobilize
· Provide professional treatment in the privacy of the patient’s
home
· Help arrange for equipment in the home and to teach the use of the equipment
to
provide
the
safest care possible
· Provide professional treatment in the privacy of the patient’s
home
· Define Therapeutic Activities
· Provide self-care management
· Treat varicose veins and ulcers
· Provide excercise and endurance training
· Help with incontinence & peroneal muscular atrophy
· Treat contractures of joints and ankylosis
· Help with back aches & lumbago
· Provide excercise and endurance training
· Provide for orthopedic aftercare

MEDICAL SOCIAL WORK

Social
Workers can:
· Help a patient and their family
with planning for long-term health care
· Help a patient and their family find other resources for assistance
within the community
· Help a patient remain independent in the home for as long as possible
